Under the leadership of Chancellor Otto von Bismarck, Germany introduced the world’s first national health insurance in 1883. The aim was to provide workers with financial protection in case of illness, preventing them from falling into poverty due to lost income from sickness. This was part of Bismarck's broader welfare reforms, which included pension and accident insurance.

The initial plan was primarily for industrial workers, but over the decades, the system expanded to cover more of the population, including employees in various sectors, students, and the self-employed. Today, Germany’s healthcare system is one of the most comprehensive and equitable systems in the world.

B. Private Health Insurance (PKV)

Private Health Insurance (PKV) is available to a smaller proportion of the population. Around 11% of Germany’s population is covered by PKV. The system is designed for people with higher incomes, the self-employed, and civil servants.

Key Features of PKV:
  1. Premiums Based on Risk: Unlike GKV, PKV premiums are not income-based but are determined by an individual's health status, age, and the level of coverage selected. As a result, younger and healthier individuals often pay lower premiums, but premiums can increase as they age or if they develop health issues.

  2. Comprehensive and Flexible Coverage: PKV offers a higher level of flexibility and additional options compared to GKV. Those covered by PKV can choose the type of insurance they want, including private rooms in hospitals, quicker access to medical specialists, and other additional services such as dental care and alternative treatments.

  3. No Free Family Coverage: In contrast to GKV, PKV requires separate insurance for each family member. This can result in higher costs for families, particularly those with children.

  4. Eligibility Requirements: PKV is available to those whose income exceeds the threshold set for GKV (currently €69,300 annually). Self-employed individuals, freelancers, and civil servants are also eligible for PKV, regardless of their income.

3. Healthcare Services Covered by Health Insurance in Germany

Both GKV and PKV cover a wide range of healthcare services, including:

  • Preventive Care: GKV provides coverage for preventive services such as vaccinations, cancer screenings, and health check-ups. PKV often provides additional preventive services and may cover more alternative treatments.

  • Dental Care: GKV covers basic dental care, including regular check-ups and fillings, but more extensive procedures like dental implants may not be fully covered. PKV offers more extensive dental coverage, depending on the plan.

4. Funding and Contributions to the Health Insurance System

The German healthcare system is primarily funded through contributions from individuals and employers. Employees pay their share of the health insurance premium directly from their salary, and employers contribute an equal amount. Germany continues to face the challenge of making its health insurance system sustainable in the face of demographic changes and rising healthcare costs. Efforts are also being made to reduce bureaucracy and improve access to care in underserved areas.

7. Health Insurance for Expats and Foreigners in Germany

Expats and foreign residents in Germany must have valid health insurance coverage. EU citizens can use their European Health Insurance Card (EHIC) temporarily, but they are required to switch to either GKV or PKV if they plan to stay longer than three months.
